NHS Athletics “White Card” Procedures

The coach will not allow you to practice if you have not completed all of the above. We no longer issue white cards to student-athletes, as the information is readily accessed online by the coaches.

You MUST have your IHSAA Physical Evaluation Form on file in the NHS Athletic Office BEFORE you can participate and begin your first official day of practice.

PLEASE NOTE:  You must also fill out five IHSAA online forms on RankOne.

  • Go to RankOne Sport
  • Select “Electronic Participation Forms” in the bar at the top of the screen
